When Your Back Hurts but Your MRI Is “Normal”

The Surprising Truth About MRI Findings
One of the most eye-opening things we have learned from research is that many people with no back pain at all have “abnormal” findings on their scans. When you put people with healthy, pain-free backs into an MRI machine, you often see:
- Disc bulges
- Disc degeneration or “wear and tear”
- Small tears in the discs
- Arthritis-like changes
These changes are often a normal part of aging, a bit like getting wrinkles or grey hair. They may sound scary on a report, but for many people they do not cause any pain at all.
The opposite is also true: some people have severe, life-limiting back pain and yet their MRI report says everything looks “unremarkable” or “within normal limits.” This shows us that the scan and the pain do not always match up. Pain is influenced by many more factors than just what we see on an image.
So What Is Causing the Pain?
If the scan is normal, you might ask, “Then why does my back still hurt so much?” There are several possible reasons, and often more than one is involved at the same time.
1. Soft Tissue Pain: Muscles, Ligaments, and Tendons
The soft tissues that support your spine — your muscles, ligaments, and tendons — can be a major source of pain. They can become strained, tight, overworked, or irritated.
While MRI can see big tears or major injuries, many everyday soft tissue problems do not show up clearly, especially if you are lying still in the scanner. You can have very painful muscle spasms or trigger points and still have a “normal” MRI.

Why Doctors Still Order MRIs
If scans do not always match pain, you might wonder why doctors order them at all. There are very important reasons.
The main job of an MRI in back pain is to rule out serious problems, sometimes called “red flags,” such as:

- Infections
- Certain types of tumours
- Severe pressure on the spinal cord or nerves that might need urgent treatment

That does not make the pain less real, but it does change how we treat it. The focus can shift from “finding the broken part” to helping your whole system calm down, get stronger, and move better.

If your pain is not caused by a clear structural problem, then treatments that focus only on structure — like certain surgeries or injections — may not give lasting relief. The encouraging news is that non-structural causes of back pain often respond well to a broader, more whole-person approach.
Helpful options may include:
- Physiotherapy and movement-based care — to improve strength, flexibility, and confidence in movement, and to calm sensitive tissues.
- Pain neuroscience education — learning how pain works in the body and brain, so it feels less mysterious and frightening. Understanding pain can actually reduce how much it hurts.
- Graded activity and exercise — slowly and safely increasing what you do, step by step, so your body and nervous system can adapt without becoming overwhelmed.
- Psychological approaches, such as CBT or ACT — talking therapies that help you manage thoughts, emotions, and behaviours around pain, reduce fear, and build coping skills.
- Improving sleep — better sleep helps your brain and body regulate pain signals, repair tissues, and handle stress.
- Stress management — techniques like breathing exercises, relaxation, mindfulness, or enjoyable hobbies that help bring your system out of “constant alarm” mode.
- Manual therapy — hands-on techniques such as gentle joint movements, soft tissue work, or massage, often used alongside active exercise.
No single treatment works for everyone. The best results usually come from combining several of these approaches into a personal plan that fits your life and values.
The Importance of Being Believed
Being told “your scan is normal” without a clear explanation can feel invalidating. You might feel brushed off, or worry that your healthcare team thinks you are exaggerating.
Your experience matters. Pain is always real, whether or not it shows on a scan. A good healthcare team will listen to your story, ask about your life, your worries, your sleep, and your goals — not just your images and test results.
If you have felt dismissed in the past, it is okay to say so and to seek out professionals who take the time to explain things and include you in decisions about your care.
Questions to Ask Your Doctor
When you next see your doctor or healthcare provider, you might find it helpful to bring some questions with you. Here are some ideas:
- If my MRI is normal, what do you think might be causing my pain?
- Could my muscles, joints, or nervous system be contributing, even if the scan looks fine?
- Can you refer me to a physiotherapist or a pain specialist who understands long-lasting pain?
- Are there pain education programmes, workshops, or reliable online resources you recommend?
- What can I safely do to stay active, and what should I avoid (if anything)?
- How can we work together on sleep, stress, and mood, since I know they affect pain?
- Would keeping a pain diary help us spot patterns in my pain and triggers?
A pain diary can be as simple as jotting down when your pain is better or worse, what you were doing, your stress levels, and how you slept. Over time, this can uncover useful patterns and help guide treatment.
